Dear Emma- A painful lump after trauma to the breast can certainly be a hematoma. Other causes may include abscess (if the skin was traumatized and let bacteria in), or fat necrosis--which often feels like a hard mass. It sounds as if you are a few days out. The best management at this point is good support (you can also wear a stretch bra to bed to ease the discomfort with movement), pain medication, and time. If the lump and/ or pain persist for more than a few weeks, please have this evaluated. It is sometimes true that a minor trauma leading to bleeding and/or a painful mass reveals an underlying, occult breast cancer. While this is low on the list, it is included in the differential. Good luck. I hope this heals up soon. Hannah Brooks, MD,FACS

I recently bumped my breast on the corner of a cabinet, resulting in mild discomfort. The next day I noticed increased pain, and a lump beneath the surface, approximately 2 x 4 cm. There is no external bruising. It has now been 4 days, and the lump and pain haven't changed. Could this be a hematoma, or something else? Does it require treatment by a doctor, or do I just need to wait a little longer? If time is all that's needed, what should I do to treat it at home? Thanks for your help.
