From: BDO - London Sent: 10 October 2002 08:42 To: 'anonymous@obgyn.net' Subject: Vaginal Bleeding, Fibroid, Cyst, FH & LSH > laparoscopy & hysteroscopy on Tuesday!

I am a 54 year old, normally healthy woman!.

HISTORY Had several years painful, heavy periods and laparoscopy & hysteroscopy in 1997 which were OK

Last year seemed to be menopausal, with irregular periods. The a few days light watery bleeding followed by a light period and then the watery bleeding continued. I saw my GP who gave me medication to stop the bleeding, and I had a Pelvic ultrasound. The results were "Hetrogeneous, attenuating, anteverted uterus seen measuring 76x43mm. Uterine texture appearances consistent with fibrotic changes, nut no obvious fibroid mass delineater. Appearances of both ovaries appear of small in size with some follicular pattern. Rt Ov 24x13x17mm Lt Ovary 27x12x11mm. Appearances are suggestive of ? menopause. No pelvic masses, cysts or free fluid seen.

My GP suggested I wait as once periods stop fibroids likely to go away.

PRESENT

Had no periods for several months, even 2 months of hot flushes which the stopped. In August had a light period with watery vaginal bleeding before and after which stopped after a couple of weeks. In September the watery bleeding started again and have had this for 4 weeks now, together a small degree of menstrual type discomfort and occasional 'shooting pains' in the cervix or uterus, also occasional pains left and right side (ovaries?). Have seen consultant and am having a laparoscopy and hysteroscopy next Monday. The 'trans-abdominal & vaginal ultrasound pelvis' showed "Uterus is enlarged, measuring 10cm in longtitudinal dimension and 5.9 cm fibroid in fundal region. Endometrial lining appears fairly homogeneous, with a well demarcater border, but appears thickened with a max thickness of 1.5cm. Rt Ovary normal in size & appearance. Lt ovary contains 2.5cm unilocular cyst which could be a functional cyst.. No free fluid or space occupying lesion in pelvic capacity.

Also blood test showed slightly increased 1 tumour marker (don't know which one).

The consultant is puzzled that blood test in March 2001 showed LH 58.6, FSH 96.2 ("definitely menopausal), and Sept 2002 LH7.9, FSH 21.1 ("not menopausal"). Says he has not really encountered this before.

QUESTIONS Any ideas what my problem could be. What is the likelihood of a cancer?. Why have LH FSH levels changed back. What is the maximum GP is likely to take out during laparoscopy & hysteroscopy?. Am due to go on holiday next Friday - is this still a possibility?

Sounds like it might be endometriosis. Check with your physician

The TSH is the more reliable test. In fact it is the only one that I get. If you don't have enough circulating T-4, the TSH will be elevated.

There are many good doctors who can do this type of surgery. We don't keep any sort of list on who deals with what insurances. You may wish to look in your insurance book and see who IS on your plan and try to check these professionals out.

Hope this helps and write back anytime for more information.

To whom it may concern,

I was using the birth control pill Demulin until I relocated in Australia for work. No one has been able to give me an equivalent pill (they apparently don't have this type here). I have had so many problems with other pills that I do not want to try any others. Do you have any suggestions?

I am 36 years old and I have a 2 year old daughter (no problem with pregnancy). I began to miscarry 2 weeks after conceiving. The only reason I found out is that I had persistent bleeding -- 4 days bleeding, 3 days ok, 4 days bleeding, 3 days ok, 4 days bleeding. Since the 1st occurrence of bleeding happened at the time of my next period I did not suspect anything. Because of the persistent bleeding I had a blood test which indicated I was pregnant. I am no longer pregnant (confirmed by blood test) and I would like to try to conceive as soon as possible. Is there any medical reason (e.g., higher risk of miscarriage or uterine lining not ready) to wait? I have read some of the information and it seems most suggest waiting a month. I would prefer to start now. What do you think?

I had 3 miscarriages before the birth of my daughter and 1 after. When I got pregnant with my daughter I was on the pill. They tested my progesterone level and it was an 8. They gave me a progesterone supplement and I was able to carry her full term.

For all of my miscarriages, I was not on the pill. My first miscarriage was at 12 weeks and they happened earlier and earlier in the pregnancy from then on. I believe I miscarried last month as well but the pregnancy wasn't confirmed. Could my progesterone level be so low that I can't carry the pregnancy long enough to even confirm it?

Guidelines for posting to this forum:

1. Please do not use abbreviations without defining them initially. For example, a sentence should read "I'm trying to conceive (TTC)," not "I'm TTC". Posts that are too difficult to interpret will be skipped. This may seem simple but many abbreviations used by posters are not approved medical abbreviations and are unknown to the health professionals.

2. Please do not refer to other websites which require the doctor/nurse to look at your temperature chart, etc. In order to help the most people in the time allotted, the presentation must be concise and to the point. Remember that the health professionals are volunteers with private practices and families. They do this out of the goodness of their hearts.

3. Please state your age and number of pregnancies and children (there may be 3 pregnancies and only one living child but 2 miscarriages) in the beginning of each post, as this is frequently important. Do not assume that the various physicians either know or remember your entire history because you may have posted before.

4. There are many threads that are posted and answered repetitively. For this reason, they may be skipped. If your post is skipped, we encourage you to review the archives. To be fair to professionals and forum regulars, it is not appropriate to take more time with such recurrent questions. These include but are not limited to:

a. When do birth control pills become effective? After one package, in general.

b. Could I be pregnant? The answer is always "Do a home pregnancy test (HPT)."

c. Can sperm live on sheets/hands/etc? Can I get pregnant from it? The answer is "yes."

5. This is not the Home Shopping Network. Nothing, including but not limited to medications and medical supplies, is to be offered for sale on this forum unless explicitly provided for or under license by OBGYN.net. All other "offerings" will be pulled.

The doctors, nurses and other health care professionals who answer your questions want to be helpful, but they cannot and will not answer all questions due to the sheer daily volume, especially ones found time and time again in the archives. They may skip any posts at their discretion, and if you do not receive an answer within 48 hours, then you may post it again. If you feel it is either urgent or an emergency, do not repost and title it with capitals, "please respond" or other such phrases. I recently had an irregular pap smear. As a result I went in for a coloposcopy today. The doctor who did my pap and the doctor who attempted the colosposcopy noted that my vaginal 'canal" is extremely long. The colosposcopy was unsuccessful because the doctor could not see my cervix. I

am scheduled for a hysterectomy shortly. Should I insist on a successful colosophy prior to the surgery even if the cervix is to be removed?

I had a pap about 2 mos ago, (*switched from family doctor to obgyn due to getting off my parents insurance) and I told her how I'd been having severe lower back pain a day or two before my period. This past February I also had a 4 cm cyst that was caused me to have painful intercourse and then a bloody discharge and then the abdominal pain. Off and on I still have abdominal pain.I've also read that while on BCP's that cysts aren't very common. I'd been on Loestrin fe 1/20 for 2 yrs. She switched me to Loestrin fe 1.5/30,*(been on for 3 mos now.) and said that it had a higher progestrin level or something like that. She also told me to take 2 600mg calcium pills a day, and gave me a RX for Anaprox for my back pain (works pretty good). Told me to take that when I feel that I'm starting to have some back pain. She told me that she might want me to have a laparoscopy done to find out whats causing my problems with my back. Is a laparoscopy something I should have done or is there another reason for my problems? If you can help me thank you so much.

Hello! I'm 24 female with endometriosis. I was told about a year ago that I had a fibroid. I have not followed up concerning the fibroid (long story). Anyway, I have been experiencing some changes in menstration. My period always lasted a week with heavy bleeding and would come about 30-35 days apart. Recently I went for 2 months without a period, took pregnancy tests that were negative. I started bleeding and thought it was my period.. Well the bleeding was not even heavy enough to wear a pantyliner, mainly it was only there after I went to the restroom, and it only last 2-3 days. I have had cervical cancer in the past (1999) and cancer runs in my family. My 21 year old (paternal)cousin has recently been battling uterine cancer. The fibroid was diagnosed through examination and vaginal ultrasound. Is it possible that it is something other than a fibroid? What could be causing the changes in my menstration? I plan on seeing a gyn, but the normal wait is 4-6 months. Should I try to get in sooner to see one? Thanks so much for any help that you can give me. Take care. Love and Prayers, Amanda
